ONSC's Liability Waiver with Covid Protocols and Agreement


ONSC is committed to protecting the health and wellbeing of our staff and visitors. As many of our visitors are youth, ONSC staff and volunteers continue to maintain safety measures including the use of cloth face coverings while indoors or closer than 6 feet, physical distancing, hand hygiene, and enhanced cleaning protocols.


Below are the safety precautions that will be taken during ONSC’s Event to mitigate the spread of COVID-19.


Event Screening:

  • Upon arrival to the ONSC campus, participants will be screened for symptoms.

  • Temperatures will be taken of all participants upon arrival and then daily at the start of each day.

  • Participants should stay home if they:

  1. Feel unwell in any way

  2. Are awaiting COVID-19 test results

  3. Have been exposed in the past 14 days: Exposure is defined as being with someone who has COVID-19 or is suspected of having COVID-19 while you were: Within 6 feet for more than 15 minutes during a 24-hour period; Masked or unmasked; Indoors or outdoors; Had direct physical contact with the individual; Shared eating or drinking utensils; Coughed or sneezed on

  4. Individuals that are at a heightened-risk for COVID-19 should consult their doctor if there is any concern over the risk of attending an event.


Minimizing Risk during the Event or Workshop:

  • Masks are required inside any building on the ONSC campus, with the exception of personal dwellings. Masks are required regardless of age or whether someone has been vaccinated. Masks must always cover both nose and mouth unless actively eating or drinking.

  • Masks are required outside any time a distance of 6 feet cannot be maintained.

  • Workshop activities will be modified and adapted to adhere to proper distancing, with most activities, including meals, held outside whenever possible.

  • While in lodges, masks must be worn except when sleeping, at which time doors to bedrooms will be closed.

  • All shared supplies will be disinfected after each use.

  • Frequent hand hygiene will be required throughout the day.

  • Where possible, participants will be separated into groups and kept within those smaller groups to avoid any unnecessary exposure.


Zero-Tolerance Policy: 

There will be a zero-tolerance policy for anyone who does not adhere to the stated COVID-19 precautions and policies. Participants will be sent home if they knowingly violate these policies and do not correct their behavior. 


Release, Waiver, and Agreement:

Even with strict precautions and safety measures, there is still a possibility of contracting COVID-19 at camp. By attending ONSC’s Mycology Weekend Workshop and signing this release, waiver and agreement, participants acknowledge they will do their best to help prevent the spread of COVID-19 and know that there is always a risk even with training and enhanced safety measures. Therefore, we require that every participant read this document, agree to the terms, sign and date the form. By signing this form, you acknowledge ONSC’s safety measures and will uphold the safety precautions required of you.

On behalf of myself and/or my child/ward (as the case may be) and as a condition of myself, and/or my child/ward attending this workshop, I hereby acknowledge and agree to assume all risks, including COVID-19 and similar risks, known and unknown, associated with attending this workshop, EVEN IF ARISING FROM THE NEGLIGENCE OF Ozark Natural Science Center (ONSC) including its board members, employees, officers, agents, volunteers, insurers or representatives (hereinafter collectively “RELEASEES”) and I hereby agree to RELEASE, INDEMNIFY AND HOLD HARMLESS all RELEASEES from and against any and all such claims, actions, suits, damages, liabilities (and the like) arising out of or in connection with, directly or indirectly, COVID-19 and the like, including, but not limited to, costs, expenses and attorney fees.
